Frequently Asked Questions

No, we do not accept insurance. However, we are committed to providing accessible care by offering affordable options, including sliding-scale rates with graduate interns. We can also provide a superbill for you to submit to your insurance provider for potential reimbursement, depending on your plan’s out-of-network coverage. Please note that reimbursement is dependent on your plan and is not guaranteed. By working outside of insurance constraints, we ensure that you receive the time and attention necessary for the best possible care.

We are located at 832 S. Florida Ave., Suite 1, Second Floor, Lakeland, FL 33801.

Our office is in a two-story building shared with the Smith Feddler and Smith Law Office, whose signage you may notice outside. To access our office, enter the two-story side of the building and take the elevator on the left to the second floor, where you will step directly into our lobby.
Parking is located behind the building and is accessible from E. Ridgewood St. and S. Tennessee Ave.

Yes, we offer convenient virtual and telehealth appointments to meet your needs. These appointments are conducted securely and are available for eligible services. Contact us to see if telehealth is right for you.

Before your first appointment, please complete the necessary intake forms through our secure patient portal. If applicable, provide any relevant medical records. Feel free to contact us if you have questions or need assistance.

You can access our patient portal by clicking the “Patient Portal” button at the top of our website or click here. If you’re a new user, please check your email for an invitation link or contact our office to get set up.
